The digital transformation of healthcare is driving the evolution of care models toward increasingly hybrid forms of care, in which telemedicine and home-based care pathways help ensure continuity of care, greater access to treatments, and the safe use of digital technologies.
In particular, respiratory rehabilitation is an area in which remote monitoring and digital tools are helping to redefine the organization of clinical pathways, facilitating continuous and safe care management even outside of healthcare facilities.
HERE – Home Rehab was born out of this need; it is a project coordinated by AENDUO that develops a solution for home-based respiratory rehabilitation based on a medical app and a kit of devices designed for physical therapy exercises.
The solution allows patients to complete their rehabilitation program independently at home, within a digital environment designed to support reliable and secure interaction with healthcare professionals.
The system includes a tablet app designed to deliver the therapeutic program and allows the physical therapist to remotely monitor the progress of the sessions and the rehabilitation process.

Key Innovations and Technologies
• Mobile app classified as medical software for respiratory rehabilitation;
• Remote Patient Monitoring (RPM);
• Physiotherapist supervision throughout the entire rehabilitation process.
Areas of Application
• Healthcare professionals providing digital respiratory rehabilitation services;
• Patients requiring ongoing rehabilitation at home;
• Insurance providers and healthcare organizations interested in more sustainable care models.
Co-funded by Cyber 4.0,HERE contributes to the development of innovative models for home-based respiratory rehabilitation, promoting more accessible and continuous care pathways supported by secure digital technologies.
The project demonstrates how innovation and safety can go hand in hand to make healthcare more attuned to patients’ needs and more closely integrated with the work of healthcare professionals.
